The American Geriatrics Society has developed the free app for Multiple Chronic Conditions: Geriatrics Evaluation and Management Strategies (MCC GEMS) to address one of the greatest challenges in clinical care of older adults: providing optimal care for frail elders with multiple chronic conditions, or multimorbidity.

About the Guide

This guide represents collaborations with the Center for Innovations in the Advancement of Care (CIAC).  It was co-created by Lita Anglin, Nursing Research and Education Librarian and Marilyn Lopez, Geriatric Nurse Practitioner who leads the NICHE program at NYU Langone Health.
